If you need a referral for insurance purposes, your primary care physician or another provider can fill one out and fax it to us. Our fax number is 503.688.5509. We are happy to arrange that for you.

When we receive the referral form, we will call you within one business day to set up your appointment. We will then send a confirmation note to your provider once your appointment is scheduled.
